Zapier’s position is explicit. In its own article on HIPAA, Zapier says it does not sign a Business Associate Agreement. That rules it out for moving patient names, appointments or visit dates between a dental PMS and GoHighLevel. It remains usable for non-PHI tasks such as posting a new review alert to Slack. NexHealth and Weave as a contrast. Both are dental-built patient communication tools with their own PMS sync. NexHealth’s Synchronizer page lists support for Dentrix, Eaglesoft, Open Dental, Curve and Denticon among more than 70 systems. Some practices keep NexHealth or Weave for recall and reminders and use GoHighLevel only for new-patient inquiries and marketing. In that design the GoHighLevel build is smaller, and the PMS connector may not be needed at all.
What to sync, minimum. Patient ID, name, mobile, consent status, next appointment and status, last visit date, provider and location. Leave clinical notes, diagnoses and balances in the PMS unless there is a reviewed reason to move them. Fewer fields mean less PHI in the CRM and fewer mapping errors.

Per HighLevel’s HIPAA compliance article (updated 11 June 2026), the agency owner goes to Settings, then Compliance, purchases the $297 a month subscription, then signs the BAA inside HighLevel’s Documents and Contracts system. HIPAA then activates agency-wide. The subscription is non-cancellable and non-refundable, and cannot be removed or downgraded.
2. Sub-account toggle. The owner must separately enable HIPAA for each location under Sub-Accounts, the location, then Advanced Settings. That toggle cannot be reversed either. A practice whose account is run by a marketing agency should confirm in writing that its own sub-account has been switched on, not only the agency.
3. The BAA chain. HighLevel signs with the account owner. If an agency owns the account, the practice needs its own BAA with the agency. The PMS connector vendor is another business associate. Each link needs an agreement, and HighLevel’s article does not confirm workflows, funnels, Zapier or third-party integrations as HIPAA-compliant, so each connector is its own decision.
4. A2P 10DLC. Carriers require business texting over local numbers to be registered. HighLevel lists a one-time $22.50 bundle for sole-proprietor and low-volume standard brands, $64 for high-volume standard, and $10 a month for a standard campaign, all pass-through fees with no HighLevel markup.
5. TCPA consent. HIPAA and the TCPA are separate regimes. Appointment reminders and marketing offers need different consent, and the FCC revised its consent-revocation framework in September 2026, as summarised by Consumer Financial Services Law Monitor. Put a separate consent line on the intake form for each message category, and make STOP work in the system, not by staff memory.
Have a healthcare attorney review the PHI boundary and consent language. The add-on handles the vendor’s technical safeguards; the practice’s risk analysis, policies and staff training remain the practice’s job.
